Planning and Supporting

Before you come to live in Tigh a Chomainn we will talk with you about what you want to get from your life and the kind of support you might feel you need in order to achieve this whilst with us.

Your Care Manager and family might also be involved in these talks.

We will write an Individual Support Plan with you where you can identify what and who are important to you, what your aims or dreams might be, how you might manage these and how we can support you in this. (See appendix 1 ‘About Me and my support’)

We will always involve you in making decisions and support you to make choices and be independent.
You will have a keyworker with whom you can regularly meet and discuss your plans.
Your Keyworker is there to listen to you and to offer advice
Tigh a Chomainn will support your Keyworker so that they can support you.

There are weekly meetings where you can say what you think about the service and if there is anything we could do better.You will, of course, be involved in regular reviews about your support
